Our vision
Legacy Point Care envisions inclusive communities where youth and adults with complex needs, including individuals with intellectual disabilities, are fully integrated, valued, and supported to exercise their rights and responsibilities as equal members of society. Guided by principles of nurturing, cultivating, empowering, and dignity, we are committed to delivering high-quality, person-directed, community-based services that promote safety, well-being, and meaningful participation.
Our mission
Legacy Point Care provides responsive, person-centred supports to youth and adults with complex needs in accordance with applicable Ontario legislative and regulatory requirements. We safeguard individual rights, promote informed choice, and ensure the delivery of safe, effective and accountable services. Working with families, caregivers and community partners, we support individuals in achieving their personal goals, developing life skills and increasing independence.

Organisational mandate
Legacy Point Care is mandated to provide safe, high-quality, person-centred care to youth and adults with complex needs, including individuals with intellectual disabilities, in accordance with applicable Ontario legislation, regulatory requirements and sector best practices.
Policy GOV-001. Reviewed annually, or sooner where legislation, Ministry requirements or our own practice change.
How we know it is working
Every person has a written plan built from an assessment of needs and personal goals, reviewed on a fixed schedule and whenever circumstances change materially.
Documented risk assessments for each individual and each residence, with safety plans that are rehearsed rather than merely filed.
No staff member works an independent shift before completing orientation and mandatory training. Supervision is scheduled, not incidental.
Accurate, timely documentation kept in line with legislative and organisational requirements, including daily notes and medication records.
A defined protocol for reporting, responding to and reviewing incidents, including serious occurrence reporting where required.
Scheduled internal audits and program reviews that check compliance and service quality against defined indicators.
Accessible channels for individuals, families and stakeholders to raise concerns, with defined response times and no adverse consequences for complaining.
